We are a local authority working for the community, improving the lives of our residents and we are looking for a motivated and talented and Experienced Social Worker in Adult Social Care here to join our Advice and Wellbeing Hub.
Working as an Experienced Social Worker for Reading Borough Council you will be part of a friendly and supporting team. We are looking for an individual who can demonstrate great communication skills and the ability to build and maintain relationships with colleagues, young adults and health care providers.

About the Role:
The Advice and Wellbeing Hub is a busy, supportive and friendly team, working with people of all ages and backgrounds and their families and carers. The Advice and Wellbeing Hub uses a Conversations Count strengths-based model to promote choice and control and achieve excellent outcomes.
The opportunity to undertake Practice Educator Training and provide student placement at least every two years.
As an Experienced Social Worker, you will need to contribute to growth of practice (for example – representing on multi-agency meetings e.g., MARAC, ICS, or actively reviewing or developing policy)
You will need to demonstrate competencies at Experienced Social Worker level on the Professional Competencies Framework.
You will work with individuals who have been referred for support for up to 6 weeks, to maximise their independence in their own homes. Promoting choice and control and using a strength-based approach.
The successful candidate will undertake work using the Conversation Counts model, working on prevention and early support. This will include using your social works skills to undertake Care Act assessments, safeguarding investigations, Mental Capacity assessments and support best interest meetings.
You will hold a varied caseload and contribute to the team performance and positive outcomes for our residents.
The role involves working with adults with a broad range of needs including people with learning disabilities, physical disabilities, older people and supporting people with substance/alcohol misuse. It also involves providing information, advice and support to Carers.

You will work within the legal framework provided by the statute, guidance, policies and procedures.
You will need to have:
* A degree or equivalent in Social Work
* Registration with Social Work England
* Understanding of the Care Act 2014 and the Childrens and Families Act 2014

We work closely with our colleagues in Reading Borough Council, Health Professionals, Voluntary and Charity colleagues to facilitate the best possible outcomes for those people we support.
This role offers Agile/Hybrid Working – you have the opportunity to work from home and the Civic Offices in Reading but also you will need to attend face to face visits within the Reading Borough.
